Please note that we give very serious consideration to all applications, since poor discernment effects not only individuals and communities, but the wider Church as well. It is as important for you, as it is for us, to discern thoroughly before making any decisions. We therefore ask that you also give your decision full, serious, and prayerful thought before completing this form.

If you are unable to provide documentary evidence while completing the form, save it and return later, when you can upload them. These are non-negotiable, and you will be required to upload:
- A clear copy of your ordination certificate (and your consecration certificate, for bishops).
- Two written references. One from a member of the clergy and one from a lay person (member of the community).
These must include:

– how long they have known you,
– any other pertinent information, and
